APPOINTMENT TIMES

Your punctual attendance is appreciated to ensure you receive your full treatment time

Should you arrive late for your appointment, treatment will still go ahead, however only for the remainder of the scheduled time
-
On the odd occasion, due to extenuating circumstances, your practitioner may be running late
If this should occur, you will still receive the full treatment time allocated to you -
To maximise your treatment time, please aim to arrive 5-10 minutes before your scheduled appointment

CANCELLATION POLICY

Please be aware that 24 hours notice is required should you be unable to attend your scheduled appointment

This allows for patients on waiting lists to be contacted to fill your cancelled appointment slot
-
Should you not attend your scheduled appointment, or cancel within the 24-hour period leading up to your appointment, a fee will apply

MEDICARE PATIENTS
​
For those eligible to receive funding for treatment under the Chronic Disease Management (CDM) Scheme:
​

A CDM referral must be issued by your GP and provided to BodyPlus Physiotherapy prior to commencing treatment
-
Should Medicare refuse to pay for treatment, any outstanding invoices will be the patient's responsibility for payment

DVA PATIENTS
​
For those who have a Department of Veterans' Affairs (DVA) Health Card:
​
-
A DVA referral must be issued by your GP or Medical Specialist and provided to BodyPlus Physiotherapy prior to commencing treatment
-
Referrals remain active for 12 months from the date of issue
- Gold Card holders are entitled to have any condition treated under the current referral
- White Card holders are only entitled to have the specified condition listed on the referral treated -
DVA are billed directly and will pay for the treatment provided

RTWSA & INSURANCE CLAIMS
​
For those receiving treatment through a ReturnToWork SA or Third Party Insurance claim:

A current medical certificate from your GP is required (please provide a copy)

You will be required to complete regular outcome measure questionnaires throughout the course or treatment
-
Provider Management Pplans will be sent to insurers by your practitioner to request further treatment approval
-
Should your insurer refuse to pay for treatment, outstanding invoices will be the patient's responsibility for payment
-
You are responsible for attending all scheduled appointments
- Non-attendances or cancellations within 24 hours of your scheduled appointment will attract a non-attendance fee and will be your responsiblity for payment, not your insurer
